Schedule: Monday-Friday 8:30am-5:00pm; 30 minute lunch

Responsibilities:
  • Conduct investigations in response to allegations related to abuse, neglect and exploitation, or other significant events.
  • Interview persons served, staff, program leadership as part of investigation.
  • Analyze various types of documentation including financial information, timesheets, GPS, and service documents.
  • Work with program leadership and Human Resources to identify actions taken or planned.
  • Prepare clear, accurate and timely reports summarizing findings and determining whether each allegation was substantiated or unsubstantiated.
  • Identify patterns of issues related to investigations and make recommendations which may decrease repetition.
  • Support Quality Department with special projects.

Qualifications:
  • Bachelor's Degree Required in Behavioral Health/HealthCare field
  • Minimum 3 years of relevant experience, including direct care experience with youth and/or adults who have intellectual disabilities and/or behavioral health issues.
  • Experience in investigating incidents preferred.
  • Strong analytic skills and ability to present concise, objective summary of findings.
  • Excellent written and oral communication, as well as interpersonal skills.
  • Valid driver's license in good standing required.
