Adult & Child Health is an integrated healthcare provider in Central Indiana. We are a Certified Community Behavioral Health Center and a Federally Qualified Health Center look alike; we strive to provide the best in healthcare services. Providing high-quality, affordable care including primary care, behavioral health services, substance abuse treatment, mobile crisis response, community-based services, homeless outreach, school-based treatment, and more.

About this role: The Health Coordinator helps to support our mission and team so we can work in the most efficient and effective way. This position is responsible for integrated patient coordination under the direction of the Integrate Care Team Model.

The Location and hours: A&C is based in Central Indiana. For this position, we service the following counties: Indianapolis, IN or Franklin, IN. Core business hours Mon-Fri

Responsibilities include but not limited to:

  • Optimized data to identify high risk patients and engage them in care
  • Administers and records data from applicable behavioral health and primary care health screens
  • Educates patient/family about diagnostic procedures, nutrition and maintenance of health and wellness.
  • Adheres to infection control/safety guidelines and confidentiality policies.
  • Participates in the development of the shared plan of care with other team members and safely coordinates the implementation of that plan.
Requirements:
  • BSW or closely related degree from an accredited University or state licensed practical nurse
  • Three years of experience preferred
  • CPR certification
