Position Description

  • Lead multidisciplinary treatment team in developing a treatment plan for each patient and documents information in progress notes.
  • Provides individual therapy according to treatment plan.
  • Provides group counseling sessions.
  • Assures coverage of scheduled activities such as educational and therapeutic groups.
  • Coordinates the development of a therapeutic milieu through an honest, supportive and structured atmosphere.
  • Keeps informed of community treatment resources from referrals to and from the program for patients and their families as evidenced by listing service for mental health, group home, assistance programs, 12-step programs and DCFS
  • Participates in program planning (curriculum, program changes, etc.)

Job requirements section:

Experience : Previous professional experience in a behavioral health setting preferred.
