PRIMARY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS
Duties of all PHP Licensed Independent Social Workers
- Coordinate Psychiatric Care: Work closely with residents, their families, and healthcare providers to assess psychiatric needs and develop individualized care plans.
- Multidisciplinary Collaboration: Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure comprehensive and integrated care for residents.
- Assessment and Evaluation: Conduct psychosocial assessments and ongoing evaluations to monitor residents' mental health status and progress.
- Treatment Planning: Develop and implement individualized treatment plans tailored to meet the unique needs and preferences of each resident, including counseling, therapy, and medication management.
- Crisis Intervention: Provide immediate support and intervention during psychiatric emergencies or crises, collaborating with the appropriate resources to ensure resident safety and well-being.
- Education and Support: Offer education and support to residents, their families, and nursing staff regarding the management of mental health issues.
- Advocacy: Serve as an advocate for residents, ensuring their rights are respected, and their voices are heard in the decision-making process regarding their psychiatric care.
- Quality Improvement: Identify areas for improvement in psychiatric care delivery and participate in quality improvement initiatives to enhance the overall quality of life for residents.
- Documentation and Reporting: Maintain accurate and thorough documentation of assessments, care plans, interventions, and outcomes in compliance with regulatory standards.
- Professional Development: Stay informed about current trends, best practices, and evidence-based interventions in the field of psychiatric social work through ongoing education and professional development activities.
Required:
- Master's degree in Social Work from an accredited institution.
- Current licensure as a Licensed Social Worker (LPC, LPCC, LPCC-S, LSW, LISW, or LISW-S) in the state of Ohio.
- Strong clinical assessment and diagnostic skills, with the ability to effectively evaluate and address mental health concerns in older adults.
- Excellent communication and interpersonal skills, with the ability to collaborate effectively with diverse stakeholders.
- Proven ability to work autonomously as well as part of a multidisciplinary team.
- Commitment to ethical practice and adherence to professional standards of conduct.
- Proficiency in computer skills for documentation and communication purposes.
- Compassionate, empathetic, and patient-centered approach to care.
- Flexibility and adaptability to meet the changing needs and priorities of the long-term care environment.
