TheOncology Nurse Navigator coordinates the care in a timely andefficient manner, identifying and eliminating barriers to care. TheNurse Navigator is a consistent care coordinator throughout thecontinuum of care and is able to assess the physical, psychologicaland social needs of the patient enhancing patient outcomes,increasing satisfaction and reducing cost of care. This nursefunctions as an expert communicator over the telephone or inperson, providing timely, appropriate advice and/or guidance topatients who seek assistance with health care issues. Performspatient assessment/reassessment with the goal of transitioning thepatient to the right level of care, based on their individualneeds. It is a complex role requiring flexibility and independentdecision making as well as multi-focal collaboration. Navigationinvolves ranking client's health problems according to theirurgency, educating and advising clients, and making safe, effectiveand appropriate dispositions according to protocols, either inperson or by telephone. Demonstrates compassionate and caringmanner with clients in person and over the phone. Additionally, theOncology Nurse Navigator functions as a patient advocatefacilitating accessibility to care and promotes optimalpatient-centered care that supports and empowers individuals,respects individual choices and meets health care needs ofpatients. Nurse participates in the care of patients through directpatient visits, patient education, phone calls, testing, and officeprocedures. Collaborates with the providers and staff in thecoordination and management of patientcare.
Requirements
MINIMUMQUALIFICATIONS: EDUCATION: Registered Nurse,BSN in Nursing, preferred.CERTIFICATION/LICENSES: Current New Mexico NursingLicense. Current BLS certifications issued by the American HeartAssociation required. SKILLS:* Considerable knowledge of health care facilities,policies and issues, and of the full spectrum of community patientcare services * Considerable knowledge ofhealth care and social management principles* Ability to coordinate various functions and activitiesfor maximum cooperation and integration of services providers andpersons receiving care within a clinic for continuum of careservices * Ability to direct, instruct andadvise staff, and to receive and effectively react to day-to-dayproblems * Ability to utilize strongcommunication skills, both written and oral, and effectivelydemonstrate an interactive style of care coordination* Ability to understand and implement sensitivity andculture of care measures appropriate to a diversepopulation * Ability to work with variouslevels of the CHRISTUS organization including clinical, financial,administrative and medical staff. * Triagescalls received from patient or families. Addresses issues directlywhen within scope of practice using protocols where appropriate ina timely manner. * Makes appropriate andindependent / collaborative decisions for selecting specific triageplan. * Documents all interactions withpatients and providers when medical direction is given.* Develop and document patient plan of care in theElectronic Medical Records system(s) *Assists in the development, maintenance, and delivery of individualtreatment summaries and care plans for patients in accordancewith applicable standards,policies, procedures, and guidelines within the Oncology ServiceLine. * Participates in committee work anddata collection and analysis for survivorship program.* Maintain knowledge of current practices in oncology,nursing, and healthcare EXPERIENCE: Minimumof 5 years of experience in a health care setting and relatedexperience with quality, care coordination and oncologypreferred. NATURE OF SUPERVISION:-Responsible to: Clinical Nurse Manager, MedicalOncology ENVIRONMENT: - Bloodborne pathogenMultiple settings and variable unpredictable situations includinginpatient, specialty and primary care settingsPHYSICAL REQUIREMENTS: Ability to move around settingsof care. Requires close work, good vision, dexterity to write aswell as to use computer keyboard. Ability to handle stressfulsituations. Most possess and demonstrate excellent customer serviceskills.
