EssentialFunctions/Areas ofAccountability

  • Providenursing care as defined in the nurse practice act for an LPN/LVNand under the direction of a registerednurse
  • ClientAssessment:
  • Contribute to assessmentof client health status
  • Collect, report, andrecord objective/subjective client data
  • Observeclient conditions or change incondition
  • Recognize and report changes inclient condition to the case manager and physician as directed in atimely manner
  • Collect data and report signs andsymptoms of deviation from normal healthstatus
  • Assist in identification of health careproblems and in formulating list of clientneeds/problems
  • Contribute to setting realisticshort and long-term goals for assignedclients
  • Plan ofcare:
  • Follow the plan of caredeveloped by a registered nurse
  • Participate indevelopment of written plan of care
  • Recognize,understand and respect cultural, spiritual, and religious beliefs,needs, and rights to choose
  • Assist inidentification of measures to support human function and maintainhygiene and comfort
  • Maintain environmentconducive to client well-being
  • Assist inassuring access to communityresources
  • Participate in the identification ofpriorities
  • Participate in reviewing andrevising the plan ofcare
  • Implementation of the plan ofcare:
  • Carry our functions taught inan approved LPN/LVN program
  • Assist client withADL's, encouraging self-care
  • Provide directcare, comfort measures, emotional support to patients who have astable or predictable condition
  • Provide care,interventions, and communicate response of the client under directsupervision
  • Assist with rehabilitativeactivities including range of motion, alignment, proper bodymechanics, and others as allowed by state ofpractice
  • Provide an environment conducive tosafety and health
  • Administer prescribedtreatment and medications, except by intravenous route (except IVcertified LPN/LVN per advancedpreparation)
  • Perform selected IV therapyfunctions as delegated and supervised by anRN
  • Document prescribed interventions andresponses
  • Assist and provide reinforcement withclient/family/other teaching
  • Maintain safe andeffective care/environment for clients andcoworkers:
  • Consult/seek guidance from RN andothers as indicated
  • Initiate standard emergencyprocedures until RN or physician isavailable
  • Apply principles of asepsis andinfection prevention and control
  • Participate inthe development, revision and implementation of policies andprocedures
  • Recognize and respond appropriatelyto potentially unsafe situations
  • Demonstratesafe practice in the use of equipment
  • Makerecommendations on improvement ofsafety
  • Evaluate client response tointerventions:
  • Evaluate effectiveness of commontreatments/tasks, document and communicate outcomes of caregiven
  • Assist with the collection of evaluationdata
  • Contribute to modification of plan ofcare
  • Collaborate with other healthcareprofessionals in providing clientcare
  • Communicate verbally and through completeand accurate documentation in the clinicalrecord
  • Seek education/training or assistancewith areas of concern
  • Perform job in compliancewith Agency policies and procedures and professional and communitystandards:
  • Accept responsibility forprofessional nursing role
  • Follow applicablestate and federal laws and agencystandards
  • Maintain confidentiality in allaspects of the job
  • Refrain from discussingclient or staff identities, conditions, or circumstances in publicor with others who do not have a legitimate interest in theperson
  • Participate in the development,implementation, and evaluation of the Agency PerformanceImprovement Program and pertinent PIactivities
  • Attend meetings and continuingeducation offerings as required
