Responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care.
The Community Hospital Navigator RN possesses intimate knowledge of the internal referral care process, clinical review criteria, utilization management standards, clinical documentation requirements, community care regulations, and Veterans Health Administration (VHA) standards and guidelines.
The Community Hospital Navigator RN uses nursing knowledge and systems theory to assess, organize, facilitate, and guide Veterans through their full range of care options including internal/direct care in Veterans Affairs (VA) and care in the community.
The Community Hospital Navigator RN serves as a liaison to both internal and community providers and is responsible for managing the care of the Veteran throughout the consult process.
The Community Hospital Navigator RN supports the medical center's mission to improve timely access to care, empower Veterans to make more informed care decisions and ensure eligible Veterans who want to receive care in the community are referred and scheduled into the community.
The Community Hospital Care Navigator RN located at the community hospital campus is responsible for assisting with care coordination needs of all Veterans.
Acts as a liaison between the VA and the community hospital for all Veteran needs.
Attends interdisciplinary rounds at the community hospital.
Assists with VA enrollment processes if applicable for Veterans not already enrolled in VA Health Care.
Works collaboratively with their VA counterpart located at the community hospital.
Assists with coordinating care for Veterans being discharged from acute inpatient care to lower levels of care.
Provides guidance to the community hospital(s) regarding VA benefits and processes.
Collaborates with various services within the VA to coordinate both inpatient and outpatient services for Veterans discharging from acute care.
Demonstrates knowledge in using VA and community resources to provide the highest quality of care in the most cost-efficient settings.
Assists with facilitating transfers from the community hospitals to VA Medical Centers when clinically indicated.
Provides notification and discharge summary documentation to Patient Aligned Care Teams (PACT) upon Veteran discharge from a community hospital.
Works collaboratively with the Medical Support Assistant(s) who process the emergency care notifications to ensure clear, thorough communication.
Possesses thorough knowledge of utilization management processes, clinical documentation requirements, current standards of care, and compliance guidelines.
Assists with data collection and analysis when appropriate.
The Community Hospital Navigator RN will comply with VA-mandated use of health information solutions that serve to store, exchange, and track referral care information.
The Community Hospital Navigator nurse will work collaboratively with the Office of Integrated Veteran Care to triage Request for Services (RFS) to promote in-house service access.
The Community Hospital Navigator RN will utilize delegation of authority, if applicable, to function at their highest level of role performance and align high-quality care with key performance indicators to ensure business practice standards meet patient care needs.
Provides peers with informal constructive feedback for improvement.
Questions clinical practices for the purpose of providing evidence-based care.
Participates in activities and strategies to sustain an evidence-based-practice culture.

Work Schedule: Full time, Monday-Friday 8:00am-4:30pm
