Position Summary:

The Case Manager provides case management coordination, individual case planning, monitoring of participants' progress, coordination, and facilitation of progress meetings as well as tracking and reporting individual and program activities. Day in the life: In a typical day, a Case Manager can expect to...

  • Individualized Case Management Services: Ensure individuals receive ongoing case planning, individualized and personalized goal setting, feedback on progress toward goals, support and follow up for making progress toward individual goals.
  • Case Management Coordination: Ensure comprehensive and optimal service delivery for all individuals by serving as the main contact and liaison for the individual and cooperating partners, persons and/or agencies.
  • Documentation: Complete and maintain essential documentation about participant needs and the services provided within established timelines and in accordance with applicable procedures, policies and requirements.

Job Pay & Perks:

  • Typical starting pay range: $24.63 - $28.00

Prior Experience & Education:

  • Relevant Experience required (1-3 years)
  • Associate Degree or Bachelor's degree in a related field preferred
