Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.



Duties

This Licensed Practical Nurse position is within the Primary Care service at the Bedford VA Healthcare System, located in Bedford Massachusetts. . It is full time at 40 hours per week.

The LPN GS-03/04/05/06, under the direct supervision of the RN, MD, DO, NP or PA is held accountable for delivering direct patient care to adult through geriatric aged patients. He/she recognizes the age related needs and demonstrates knowledge and care appropriate to that age group. He/she considers the cognitive, physical, emotional, and chronological maturation needs of the patients, demonstrates competence and correctly provides appropriate care for that age group or groups.

Duties include, but not limited to:

  • Participates in daily huddles, weekly pre-planning and monthly team meetings with core members of the PACT team.
  • Coordinates care by working across care settings to assist patients and ensure they receive the necessary care.
  • Relays relevant information to PACT/interdisciplinary team.
  • Uses appropriate data sources for panel management.
  • Ensures release of information (ROI) is obtained and properly documented in the health record.
  • Utilizes clinic slots appropriately, maximizing patient flow.
  • Collaborates with internal and external services to facilitate care transition in order to effectively meet the patient's needs.
  • Properly utilizes all methods of care delivery to include video and telehealth.
  • Collaborates with the Nurse Manager; creating a positive work environment which enhances care delivery and professional development of clinically competent staff.
  • Manages consults and patient inquiries (electronic or telephone) expeditiously.

The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be flexible and sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete Employee Health requirements.
