The position is located in the Outpatient Clinic Departments, at the Fort Peck Service Unit. One position will be filled at each of the following locations: Poplar, Montana & Wolf Point, Montana. The incumbent of this position will report to the Director of Nursing.
Duties
- Assess the patient care needs for the Primary Care Medical Home (PCMH) patients to include triage of acute issues for determination of appropriate level of care.
- Provides direct care during face to face visits and telephone visits.
- Develops a plan of care for individual patients utilizing nursing diagnosis.
- Consults with primary care providers and other healthcare professionals on patients to coordinate diagnostic and therapeutic regimens for PCMH patients.
