The 149-bed medical center is a nonprofit, acute-care facility, where more than 300 primary care and specialty physicians are committed to providing exceptional healthcare services to our community with dignity, compassion and respect. Statement of Purpose: The Substance Abuse Counselor/Recovery Coach (SAC/RC) will work under the direction of the Manager of Behavioral Health (LICSW). The SAC/RC will function as part of the Substance Abuse team to provide substance abuse education and assist the patient in the effective navigation of community substance abuse resources. The SAC/RC is expected to be in the community at minimum 50% of the time, working with patients to provide support in navigation. The SAC/RC is responsible for utilizing communication skills to seek information as necessary from providers, community mental health # substance abuse organizations, and families to best serve our patients, which will enable the SAC/RC to identify problems related to the total well-being of the patient. # The SAC/RC must have knowledge of community substance abuse resources, addictions treatment, and the ability to work with state agencies such as DMH and DCF to ensure favorable patient dispositions.ability to work efficiently, have great patient interaction skills, and a sense of urgency are a must.
