Position Overview: We are seeking a dedicated Nurse Practitioner to join our care team in Florida. You will play a pivotal role in providing comprehensive healthcare services to patients throughout their healthcare journey. This position offers the opportunity to make a significant impact on patient outcomes and promote continuity of care.

In-Home Assessments (IHA): Conduct comprehensive in-home assessments for patients, including medical history reviews, physical examinations, and medication management.
Patient Assessment: Conduct thorough assessments of patients' health status, including physical examinations, medical histories, and risk assessments.
Chronic Disease Management: Manage and monitor chronic health conditions, such as diabetes, hypertension, and cardiovascular diseases, ensuring optimal disease control and patient education.
Preventive Care: Promote preventive healthcare measures, including immunizations, screenings, and lifestyle modifications, to improve patient health and prevent future illnesses.
Provide patient education on disease management, healthy lifestyles, and self-care practices to empower patients to take an active role in their health.
Collaborative Care: Collaborate closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and seamless transitions for patients.
Primary Care Provider of Record: Serve as patients’ primary care provider.
Maintain accurate and up-to-date electronic health records (EHR) to track patient progress, treatment plans, and interventions.
Bedside Manner: Exhibit exceptional bedside manner, fostering trust and rapport with patients and their families through genuine care and empathy.
Quality Assurance: Participate in quality improvement initiatives and adhere to healthcare regulations and standards to maintain the highest level of care quality.
After-Hours On-Call: Participate in after-hours on-call and conduct longitudinal care and IHA visits as needed.
Travel: Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner.
Preferred two years of experience in a primary care or longitudinal care setting.
Current Basic Life Support Certification for health care providers.
Proficiency in electronic health record (EHR) systems.
