Provide and coordinate care to the high-risk VAD patient population across the continuum in collaboration with CT Surgeons, Advanced Heart Failure Cardiologists, and other members of the multidisciplinary team. 1-2 years Cardiology and/or Critical Care experience preferred. Advanced Heart Failure/VAD experience preferred.

Details:

  • Round on inpatients post VAD implantation/readmitted VAD patients
  • Outpatient clinic
  • Advanced assessment, planning and coordination of care, modification of treatment plan as indicated and the monitoring/evaluation of performance improvement activities and outcomes
  • Actively involved in patient and community education for VAD patients and their families on how to care for themselves with timely feedback on their progress
  • Provide ongoing education and support to acute care staff (ED, OR, ICU, Cardiac Surgery, etc)
  • Integral part in preparing and maintaining Joint Commission Accreditation to include but not limited to survey, leading weekly multidisciplinary team meetings and database maintenance
  • Research opportunities available
  • Monday-Friday (7A-5P), rotating hours
  • Weekends, 1/7 reimbursed separately from base salary
  • Some Holidays required, rotating basis
  • Call required, rotating basis
