You will direct, facilitate, and coordinate admissions to our facility. We are an amazing team that works hard to support each other and are seeking a phenomenal addition like you who feels patient care is as meaningful as we do. You will initiate the nursing component of the Interdisciplinary Assessment for all new admissions.
You will review transferring information from other facilities and assimilates that with assessment data to assist team members to develop the plan of care.
You will act as a liaison with admissions, pharmacy, acute care facilities and other departments to facilitate a smooth admission process.
You will coordinate the patient's care and identified needs with other services and departments throughout the hospital.
Nursing Diploma or ASN required; One to two years’ experience with an acute care facility.
